How to Enroll

Thank you for your interest in our programming. We would love to talk with you about the many options we have for participation. Here are the steps to enroll and learn more.

Step 1 - Contact us

For details on our classes and tuition, fill out the form on the right, email programs@fieldstonefarm.org , or call us at (440) 708-0013, ext. 145.

Step 2 - Fill out an application form

Fill out application and health forms. The button below will direct you to a PDF to download.

Step 3 - Send us your completed application

Send your completed application forms to Fieldstone Farm one of the following three ways:

  1. Scan and email to programs@fieldstonefarm.org.
  2. Mail to Programs, Fieldstone Farm, 16497 Snyder Road, Chagrin Falls, OH 44023

Step 4 - We'll be in touch

We currently have a waiting list. Once we receive your application, we will put you on the waiting list. We will contact you to schedule an assessment. We complete assessments of all potential students to evaluate each student’s individual needs and ensure the best fit into our program. The assessment lasts about an hour and costs $40.
