Registration
Thank you for your interest in our programming. We have many options we have for participation. Here are the steps to register and learn more. If you are a current student and need to fill out your annual paperwork, skip to Step 2.
Step 1 - Contact us
If you are a new student and would like details on our classes and tuition, fill out the inquiry form on this page (to the right for desktop, below for phones and tablets), email programs@fieldstonefarm.org, or call us at (440) 708-0013, ext. 145.
Step 2 - Fill out an application form
If you are a new student or a current student and would like to register, fill out the application and health forms. The buttons below will direct you to a PDF to download and then fill out. Once you have the form filled out you may either scan and email to programs@fieldstonefarm.org OR mail to Programs, Fieldstone Farm, 16497 Snyder Road, Chagrin Falls, Ohio 44023. Current students may also drop off the form at your lesson. Current students will skip Step 3 and we will see you in class.
Step 3 - We'll be in touch
We currently have a waiting list for new students. Once we receive your registration, we will add you to the waiting list and contact you to schedule an assessment. We complete assessments of all potential students to evaluate each student’s individual needs and ensure the best fit into our program. The assessment lasts about an hour and costs $40.
